Who We are
D.I.S.E
D.I.S.E has over 25 years of combined experience within the care sector, spanning disability, nursing, and social services. This experience encompasses direct support work, operations, managerial and director roles.
D.I.S.E specializes in individual goal-oriented disability support, working closely with you to customize our services for an optimal and enjoyable experience.
Assist - Personal Activities (0107)
Assistance with personal tasks of daily life in your home, to support you to live as independently as possible. This can include overnight support, and personal care tasks.
Assist - Travel & Transport (0108)
Whether it's to school, work, Tafe or just to complete your daily activities D.I.S.E can transport you.We will organise a qualified and trustworthy support worker to pick you.
Assist -Community Participation (0116)
Our ICP program is designed to promote purposeful relationships, in which mentors assist you to access mainstream activities that directly relate to your NDIS plan goals.
Assit - Development Life Skills (0117)
Helping you become more independent with daily skills, including - public transport awareness, financial skills and budgeting, pursuing studies and/or work, drivers license achievements, cooking and other tasks.
Participation in Community, Social & Civic Activities (0125)
D.I.S.E can assist you to plan and access social and recreational activities within your community with a support worker of your choice. Together, you will set and achieve goals.
